Q: There were no instructions for attaching the holder to the bike. There are 2 screws. Are there any instructions somewhere?

A: 99% of bicycles are are equipped with the screws. They are mounted to the frame cross members. Either the one under the seat that runs to the crank assembly area. Or the one that runs from the crank assy to front handle bar socket. They will normally be a hex head requiring an Allan key set

Barely adequate. The cage tends to loosen unless one applies just the right torque to its bolts. The bottle does not do that good a job of keeping its contents cool.
