Wall insert completely sheared.

I followed the directions precisely; pre-drilling, tapping, etc. While tightening the screw with a screwdriver, the screw became bound in the insert, and the insert completely sheared (far before the screw was near to being tight). I considered that this might just be a material or manufacturing flaw, so I tried again with a second anchor, and the same thing happened. Now I have two 1/2" holes in my newly painted wall that I'll have to spend half a day repairing, and I may have to use a hacksaw to remove the faulty anchors from my hanging bracket. Very disappointed.
